GAUGE: 12 sc + 11 rnds = 4 in. (10 cm). BE SURE TO CHECK YOUR GAUGE. When you match the gauge in a pattern, your project will be the size specified in the pattern and the materials specified in the pattern will be sufficient. If it takes you less stitches and rows to make a 4 in. [10 cm] square, try using a smaller size hook or needles; if more stitches and rows, try a larger size hook or needles.

HAT
Beginning at top of Hat with A, ch 4. Join with sl st in first ch to form a ring. Place marker for beginning of rnd, move marker up each rnd.
Rnd 1: Work 5 sc in ring, join with sl st in first sc.
Rnd 2: Ch 1, 2 sc in each sc around, join with sl st in first sc - 10 sc.
Rnd 3: Ch 1, (sc in next sc, 2 sc in next sc) 4 times; join with sl st in first sc - 15 sc. Change to B. Do not fasten off A.
Rnd 4 (Worm rnd): Ch 1, *sc in each of next 2 sc, in next sc work sc, ch 5, sl st in 2nd ch from hook and next 3 ch, sc in same sc (worm made), rep from * around; join with sl st in first sc. Fasten off B.
Rnd 5: With A, ch 1, (sc in next 3 sc, 2 sc in next sc) 5 times; join with sl st in first sc - 25 sc.
Rnd 6: Ch 1, (sc in next 4 sc, 2 sc in next sc) 5 times; join with sl st in first sc - 30 sc.
Rnd 7: Ch 1, (sc in next 4 sc, 2 sc in next sc) 6 times; join with sl st in first sc - 36 sc.
Preemie size only:
Rnds 8-18: Ch 1, sc in each sc around; join with sl st in first sc - 36 (42, 48) sc. Fasten off A.
0-6 month size only:
Rnd 8: Ch 1, (sc in next 5 sc, 2 sc in next sc) 6 times; join with sl st in first sc - 42 sc.
Rnds 9-21: Ch 1, sc in each sc around; join with sl st in first sc - 36 (42, 48) sc. Fasten off A.
6-12 month size only:
Rnd 8: Ch 1, (sc in next 5 sc, 2 sc in next sc) 6 times; join with sl st in first sc - 42 sc.
Rnd 9: Ch 1, (sc in next 6 sc, 2 sc in next sc) 6 times; join with sl st in first sc - 48 sc.
Rnds 10- 24: Ch 1, sc in each sc around; join with sl st in first sc - 36 (42, 48) sc. Fasten off A.
FINISHING
Edging Join B with sl st along last rnd of Hat. Turn to work from WS, ch 1, *sc in each sc around, working a worm (same as for Rnd 4) centered between worms of Rnd 4. Join with sl st in first sc. Fasten off.

I made a set of these for baby twins in the family. The hats looked absolutely adorable on them!
The first worm round #4 especially, and following round #5 were a little dicey for me, but after figuring it out, it was a breeze. I also as the former reviewer wrote, added more worms to the last round after enlarging the hat for larger heads, keeping with the number count.
